Agreement Between Karaayo Health Solutions Private Limited and Doctor
This agreement is entered into by and between Karaayo Health Solutions Private Limited, having its registered office at [Address], hereinafter referred to as "Company," and the Doctor, practicing at [Clinic Address], hereinafter referred to as "Doctor."
Purpose
The Company aims to promote Ayurveda among its customers and provide a platform for registered doctors to connect with a broader patient base.
Terms and Conditions
Doctor's Commitments
ConsultationThe Doctor agrees to provide consultations to the Company’s customers. While the consultation will be free of charge, the Doctor may charge customers for the cost of medicines.
Panchakarma TreatmentThe Doctor agrees to offer Panchakarma treatments to the Company’s customers at a discounted price.
Liability
The Doctor will be solely responsible for the health and well-being of the patient (Company customer) during and after the consultation or Panchakarma treatment.
The Company will not be liable for any health issues that occur during or as a result of the consultation or Panchakarma provided by the Doctor.
Exclusivity
The Doctor agrees not to sign similar agreements with any other company offering the same or similar services as those provided by the Company during the tenure of this agreement.
Agreement Tenure
This agreement is valid for one year from the date of signing.
Declaration
I, the Doctor, agree to abide by the above terms and conditions.
